This can happen when you're running Windows Vista with UAC turned on (and you are running in this secure, recommended way, right?) but you're not running SFTMIME from an elevated command prompt.  The SFTMIME command requires elevated privileges when used with certain commands such as the “LOAD” switch, so to be sure you don't ever see this message then when launching the command prompt that is used to run the “SFTMIME” command, right click on the “CMD” icon and select “Run as Administrator”.  That way you'll be sure you have the permissions you need.
